H4U (Health for You) เป็นแอปพลิเคชันที่พัฒนาโดยกระทรวงสาธารณสุข ใช้เพื่อแสดงประวัติสุขภาพส่วนบุคคลหรือสถานะสุขภาพของผู้ใช้งาน รวมถึงใบรับรองสุขภาพที่เกี่ยวข้องกับ COVID-19 แอปนี้ได้รับการพัฒนาและเผยแพร่โดยสอดคล้องกับการปกป้องข้อมูลส่วนบุคคลของผู้ใช้และกฎหมายปัจจุบัน
แอปนี้มีการแสดงข้อมูลที่เกี่ยวข้องกับ COVID-19 โดยระบบจะแสดงสถานะการฉีดวัคซีน ผลการตรวจทางห้องปฏิบัติการ(ATK/RT-PCR) ประวัติการติดเชื้อ และใบรับรองสุขภาพเพื่อให้ผู้ใช้งานสามารถใช้ข้อมูลเพื่อยืนยันการเข้าร่วมกิจกรรมต่าง ๆการใช้แอพนี้เป็นไปโดยสมัครใจทั้งหมด และไม่จำเป็นต้องเข้าถึงบริการที่เกี่ยวข้องกับ COVID-19 ของประเทศไทย
แอปนี้ไม่ใช่แอปสำหรับติดตามตัวบุคคล
ข้อมูลที่รวบรวม
ข้อมูลต่าง ๆ จะช่วยให้กระทรวงสาธารณสุขของไทยเข้าใจสถานการณ์ด้านสุขภาพและการควบคุมโรค รวมถึงสถานการณ์การฉีดวัคซีนในปัจจุบัน ประสิทธิผลของวัคซีน และเพื่อติดตามผลข้างเคียงของวัคซีนภายใต้การอนุมัติของกรมวิทยาศาสตร์การแพทย์สำหรับห้องปฏิบัติการ การใช้ยาฉุกเฉินของสำนักงานคณะกรรมการอาหารและยาของไทย พ.ศ. 2555 และพระราชบัญญัติโรคติดต่อ กรมควบคุมโรค พ.ศ. 2558
แอปพลิเคชันนี้รวบรวม:
1. ข้อมูลหมายเลขประจำตัวประชาชน ใช้ในการจับคู่และแสดง ประวัติการฉีดวัคซีน ผลการตรวจทางห้องปฏิบัติการ ประวัติการติดเชื้อ และใบรับรองสุขภาพจากฐานข้อมูลของ ประเทศ เพื่อระบุใบรับรองสุขภาพได้อย่างถูกต้อง จึงรวบรวมข้อมูลต่อไปนี้ ชื่อ นามสกุล เพศ คำนำหน้า และหมายเลขโทรศัพท์ เมื่อคุณเข้าถึงบริการด้านการดูแลสุขภาพโดยใช้แอปนี้ หมายเลขโทรศัพท์จะถูกรวบรวมเพื่อกู้คืนรหัสผ่าน และเพื่อให้บุคลากรทางการแพทย์ของเราสามารถติดต่อ เพื่อวัตถุประสงค์ในการติดตามผลเหตุการณ์ไม่พึงประสงค์จากการฉีดวัคซีนเท่านั้น
2. รวบรวมที่อยู่ (จังหวัด อำเภอ รหัสตำบล) เพื่อเลือกจัดบริการสุขภาพและนัดหมายบริการสุขภาพ
3. มีการกรอกรหัสหลังบัตรประจำตัวประชาชนเพื่อยืนยันตัวตนกับ ฐานข้อมูลกระทรวงมหาดไทยเท่านั้น เราไม่ได้รวบรวมข้อมูลเหล่านั้นและทิ้งไปเมื่อสิ้นสุดกระบวนการ ขั้นตอนนี้มีความสำคัญอย่างยิ่งในการป้องกันการฉ้อโกงข้อมูล ระบุตัวตน
4. ฟังก์ชั่นตรวจสอบบัตรผ่านสุขภาพดิจิทัลสำหรับโควิด-19 ใช้กล้องเพื่อสแกนรหัส QR บนบัตรผ่านที่ออกโดยรัฐบาลไทย และอนุญาตให้ผู้ใช้ตรวจสอบสถานะของบัตรสุขภาพ ฟังก์ชันตัวตรวจสอบทำหน้าที่เป็นตัวยืนยันเท่านั้นและไม่ได้ รวบรวมข้อมูลใด ๆ
5. การเข้าถึงกล้อง ไมโครโฟน และพื้นที่จัดเก็บข้อมูล
1) การขอเข้าถึงกล้องหรือวิดีโอ: ระบบอาจมีการขอเข้าถึงกล้องถ่ายภาพหรือวิดีโอเพื่อการบันทึกข้อมูลสุขภาพของท่าน เช่น การตรวจ ATK ด้วยตนเอง รวมถึงการวิดีโอคอลเพื่อสื่อสารผ่านแอปพลิเคชัน
2) การเข้าถึงพื้นที่จัดเก็บข้อมูล: ระบบจะมีการขอเข้าถึงและเขียนข้อมูลพื้นที่จัดเก็บข้อมูลบางส่วน เพื่อแนบรูปภาพหรือไฟล์อื่น ๆ ประกอบการบันทึกข้อมูลสุขภาพของตัวผู้ใช้งานเอง รวมถึงการบันทึกข้อมูลรูปเก็บไว้ในพื้นที่จัดเก็บข้อมูลของผู้ใช้งาน
3) การเข้าถึงไมโครโฟน: เราอาจมีการเข้าถึงไมโครโฟนและเข้าถึงเสียงของโทรศัพท์มือถือของท่านเมื่อมีการสนทนาผ่านวิดีโอคอลเกิดขึ้นในแอปพลิเคชัน
6. ข้อมูลที่เก็บไว้จะมีการเข้ารหัส
7. การเชื่อมต่อของแอปและเซิร์ฟเวอร์ได้รับการเข้ารหัสทั้งหมด
8. ข้อมูลทั้งหมดไม่ว่าจะบนอุปกรณ์หรือบนเซิร์ฟเวอร์ จะถูกลบเมื่อไม่ต้องการใช้อีกต่อไป
9. ID ของอุปกรณ์ถูกรวบรวมเพื่อระบุผู้ใช้สำหรับการแจ้งเตือนในบางฟังก์ชั่นเท่านั้น
ข้อมูลที่รวบรวมทั้งหมด จะคล้ายกับข้อมูลที่รวบรวมจากการลงทะเบียนฉีดวัคซีน สถานที่ทดสอบในห้องปฏิบัติการ (ATK/ RT PCR) และโรงพยาบาล
การแชร์ข้อมูล
1.การรวบรวมจะถูกแบ่งปันกับบุคลากรทางการแพทย์เพื่อวัตถุประสงค์ด้านสาธารณสุขเท่านั้น ข้อมูลจะไม่ถูกแชร์เพื่อวัตถุประสงค์ทางการค้าโดยไม่ได้รับอนุญาตจากคุณ
2. ข้อมูลที่รวบรวมเพื่อช่วยกระทรวงสาธารณสุขบริหารจัดการสถานการณ์สุขภาพของประเทศไทย และพัฒนาแผนส่งเสริมสุขภาพ

3. ข้อมูลจะถูกจัดเก็บบนเซิร์ฟเวอร์ในประเทศไทยและจัดการโดยหน่วยงานภาครัฐ โดยมีวัตถุประสงค์เพื่อควบคุมการแพร่ระบาดของไวรัส COVID-19 หรือดำเนินการวิจัยทางวิทยาศาสตร์เท่านั้น

H4U (Health for You) is the Thai government's official application, developed by the Ministry of Public Health, Thailand, used to display personal health record or health status of owner including health certificates related to COVID-19. The app is developed and released
This app is part of a response to COVID-19 and accesses only information collected to support COVID-19 efforts and epidemiological research on vaccination states, laboratory results (ATK/RT-PCR), infection history, and health certificate to enable people who own health information to participate in various activities.
H4U does not and cannot perform diagnostics. It makes recommendations about what to do next based on the user's history of exposure to potentially contagious users. However, the app is not a medical device and should not be used in place of a doctor.
The use of this app is entirely voluntary, and it does not necessitate access to Thailand's COVID-19-related services.
THIS IS NOT A CONTACT TRACING APP.
Collected data.
To help the Thai Ministry of Public Health understand the health situation and disease control including the current vaccination situation, vaccine effectiveness and to track the vaccine adverse effect under the Department of Medical Sciences' conditional approval for laboratory conditions, the Thai FDA's Emergency Use of Medicinal Products AD 2012, and the Department of Disease Control Communicable Disease Act AD 2015.
The app gathers:
1. Your demographic data, the citizen ID is especially important for the government to correctly match and display vaccination records, laboratory results, infection history, and health certificates from the national database. To correctly identify the health certificate, the following information was gathered: name, last name, gender, prefix, and phone number. When you have access to healthcare services by using this app, phone numbers are collected for password restoration and to allow our healthcare personnel to reach out for vaccination adverse event follow-up purposes only.
2. Address (Province, District, Subdistrict code) were collected to select for organizing health services and making appointments for health services.
3. The ID behind your citizen ID card was entered only to verify your identity with the Ministry of Interior database. We did not collect either of those data and discarded it at the end of the process. This step is especially important to prevent identity fraud.
4. The COVID-19 Digital Health Pass verifier function uses the front camera to scan the QR code on the pass issued by the Thai Government and allows the user to verify the status of the health pass. The verifier function only serves as a verifier and does not collect any data.
5. Access to camera, external storage, and audio.
5.1 Camera
The users’ CAMERA will be accessed to record health information by taking pictures.
5.2 External storage
The application requires accessing to read and write external storage on users’ devices in case of health record. For writing, the system will save the image to the users’ devices after taking pictures in ATK self-test function.
5.3 Audio
The application requests accessing to users’ devices microphone, and record audio to send to another user in video calls function.
6. The information stored on the smartphone is encrypted.
7. The app's and the server's connections are encrypted.
8. All data, whether on the device or on the server, is deleted when it is no longer needed.
9. Devices’ ID were collected to identify the users for notification in some functions.
All collected data will be similar to those collected at vaccination site registration desks, laboratory testing sites (ATK/ RT PCR), and hospital without application.
Sharing data.
1. The collected were shared with the health care personnel for public health purposes. The data will not be shared for commercial purposes without your permission.
2. Information gathered to help the Ministry of Public Health manage Thailand's health situation and develop health promotion plans.
Other information.
1. The use of personal data is governed by the Personal Data Protection Act, B.E. 2562 (2019) of the Kingdom of Thailand.
2. The Ministry of Public Health is the body in charge of collecting data and deciding how to use it.
3. The data is stored on servers in Thailand and managed by government bodies, with the sole purpose of containing the COVID-19 epidemic or conducting scientific research.
